Featuring a black and white headshot of the actor next to his vital statistics [grey eyes, brown hair, 72 inches in height, 170 pounds, etc.], signed by him in black fountain pen ink on the right side margin, back side notes card was issued to Ford on "30 Dec 1958" when he was 42 years-old, laminated.

Ford joined the United States Navy Reserve in 1958 (thus this ID card), and was
commissioned as a lieutenant commander as well as being made a Public Affairs Military Officer. He had previously served in the Marine Corps Reserves during WWII and continued on with his military service throughout the Vietnam War.
Included with a "Letter of Provenance" signed by Peter Ford, son of Glenn Ford.
