According to the plaque below the disc, "Presented To / Jake Hooker / To Commemorate RIAA Certified / Gold Sales of the More Than / 500,000 Copies of the / Arista Records / Album, Cassette and C.D. / 'Five'." Measures 13" x 17". In Very Good Minus condition with buckling and tears in the bottom right corner of the frame. COA from Heritage Auctions.
