According to the plaque in the bottom center portion, "Presented To / Polygram Records / To Commemorate the Sale of More Than / 500,000 Copies of the / Riva/Polygram Records / Long-Playing Record Album / 'Uh Huh'."

Measures 17 ¼" x 21 ¼". There are various scrapes on the frame, along with tearing on the backing paper, as well as toning on the interior and peeling on the mini-album cover, and scratches on the clear protective cover.
