Other forms include The Untouchables star Robert Stack's signed Employee Withholding Allowance Certificate, Family Affair's Johnny Whitaker's December 1965 form for General Hospital episodes, musician Bobby Troup's 1953 appearance form, British leading man (and former Elizabeth Taylor husband ) Michael Wilding's 1963 form, and Steve Martin's 1966 form to appear on TV's Dusty's Attic.
